Annual Fall Student Survey
We invite your child to participate in the annual fall student survey, which helps the Bullhead City School District gather valuable feedback directly from students about their school experience. We will administer this year’s survey during the week of October 26, 2026, and is intentionally shorter to help us better identify specific strengths and needs across our school sites.
The survey includes nine questions for students in grades 3–4 and 10 questions for students in grades 5–8. You may review all survey questions below. The questions are also included on the consent form sent home with your child.
Although we encourage all students to share their feedback, participation is completely voluntary. No action will be taken against your child, you, or the school if your child does not participate.
Your child’s participation is important! By hearing directly from students, we can ensure their voices are heard and their insights help shape the future of our school community. The more students who participate, along with parents who complete the separate parent survey, the more valuable information we can gather. This feedback will help our schools, administrators, and governing board make informed decisions, guide future planning, and identify opportunities for improvement as we continue working to provide a positive, supportive, and engaging learning environment for all students.
Please complete and return the student survey consent form by Wednesday, October 20, 2026. Be sure to check either the “Yes” or “No” box to indicate whether you consent to your child’s participation. If you do not receive a consent form or it is misplaced, please contact your school office to request another copy.
